I noticed a similar effect when I tried to run antialiasing on the highest setting, turning this setting down a notch fixed the problem for me.
May want to try turning the setting off and see if clears up for you.

Hayden, I'm seeing different issues with both v4 fixes. With 4A fix the in game HUD and inventory is stretched for me not centered. Thanks for the information Spyle311, this fixed worked for me. Just need to fix the blocky shadows now, without reducing the draw distance and/or quality.
